SYNERGY HomeCare of Plainfield (“SYNERGY HomeCare of Plainfield,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect, how we use it, who we share it with, and the choices available to you.
This policy applies to synergyplainfieldil.com and to the phone, text, and email communications we exchange with you.
1. Information we collect
Information you give us directly. When you contact us, request a consultation, submit a care inquiry, or apply for a caregiver position, we may collect:
- Name and the name of the person needing care
- Phone number and email address
- Mailing or service address
- Details you choose to share about care needs, schedule, and situation
- Employment history and qualifications, if you apply for a position
Information collected automatically. When you visit our website we may collect standard technical information such as IP address, browser type, device type, pages viewed, and referring page. We use this to keep the site working properly and to understand which pages are useful.
We do not knowingly collect information from children under 13.
2. How we use your information
We use the information we collect to:
- Respond to your inquiry and discuss whether our services fit your needs
- Schedule and coordinate care, and communicate about it
- Send appointment reminders and service-related updates
- Process and evaluate employment applications
- Meet our legal, licensing, and recordkeeping obligations as an Illinois licensed Home Services Agency
- Improve our website and services
3. Text messaging (SMS/MMS)
This section describes our text messaging practices specifically.
Consent
We send text messages only to people who have given us express written consent to receive them. You may give consent by checking the SMS consent box on a form on our website, by signing a service agreement that includes text messaging consent, or by texting us first.
Consent to receive text messages is not a condition of receiving any service from us. You can decline text messages and still become a client, and we will contact you by phone or email instead.

Message frequency varies based on your care schedule and the conversations you have with us.
Message and data rates may apply. Your mobile carrier’s standard rates apply to messages you send and receive. We do not charge for text messages.
How to opt out
Reply STOP to any text message from us to stop receiving text messages. You will receive one confirmation message and then no further texts. You may also ask us to remove you by calling 815-267-1110 or emailing us.
Opting out of text messages does not opt you out of phone calls or email, and does not affect the care you receive.

Carriers
Carriers are not liable for delayed or undelivered messages. Message delivery is subject to your carrier’s transmission and coverage.
4. How we share information — and what we never share
We share information only as described here:
- With our caregivers and staff, to the extent needed to provide and coordinate your care
- With service providers who help us operate — for example, our scheduling and client management systems, our website host, our email provider, and the text messaging platform we use to send the messages you have asked to receive — under agreements that require them to protect your information and use it only to provide services to us. These providers act on our instructions. They are not permitted to use your information for their own marketing or to share it onward.
- When required by law, including in response to a subpoena, court order, licensing inspection, or lawful request by public authorities, and to comply with our obligations under Illinois home services agency regulations
- To protect safety, where we reasonably believe disclosure is necessary to prevent harm
We do not sell your personal information.
Mobile information will not be sold or shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
All of the categories of sharing described in this section exclude text messaging originator opt-in data and consent; that information is not shared with any third party for any purpose other than delivering the messages you asked to receive.
5. How we protect information
We maintain administrative, technical, and physical safeguards designed to protect your information against unauthorized access, disclosure, alteration, and destruction. Access is limited to staff and caregivers who need it to do their jobs.
No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
6. How long we keep information
We keep your information for as long as needed to provide services and to meet our legal, licensing, tax, and recordkeeping obligations under Illinois law. Records related to client care are retained for the period required by the Illinois Department of Public Health.
Text messaging consent records — including the date, time, and the exact consent language you agreed to — are retained for as long as you remain opted in and for a reasonable period afterward, so that we can demonstrate consent if asked.
7. Your choices and rights
You may:
- Opt out of text messages by replying STOP
- Opt out of marketing email using the unsubscribe link in any marketing message
- Request access to the personal information we hold about you
- Request correction of information that is inaccurate
- Request deletion of information, subject to our legal and licensing retention obligations
To make a request, contact us using the details in Section 10. We may need to verify your identity before responding.
Illinois residents: the Illinois Personal Information Protection Act governs our handling of personal information and our obligations in the event of a data breach.
8. Cookies and analytics
Our website uses only what is necessary to operate the site and to understand aggregate usage. We do not use advertising cookies or sell browsing data. Where analytics are used, they are configured to avoid collecting more personal information than necessary.
You can set your browser to refuse cookies; parts of the site may not work as intended if you do.
9. Third-party links
Our site links to other sites, including synergyhomecare.com and our Google Business Profile. We are not responsible for the privacy practices of sites we do not operate. Review their policies before providing information.

11. Health information
If you become a client, the health information we create and maintain about you is also covered by our Notice of Privacy Practices, which describes your rights regarding that information in more detail. 12. Changes to this policy
We may update this policy. When we do, we will revise the “Last updated” date above. If changes are significant, we will provide a more prominent notice. Continued use of our website or services after an update means you accept the revised policy.

Scope of services
SYNERGY HomeCare of Plainfield is licensed by the Illinois Department of Public Health as a Home Services Agency (License #3003141) and provides non-medical adult home care services, including personal care, homemaker, and companion services. We do not provide skilled nursing, therapy, or childcare services, nor any other service requiring a Home Health Agency license. Our services are provided to adults only.
