Notice of Privacy Practices

This notice describes how information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
SYNERGY HomeCare of Plainfield is licensed by the Illinois Department of Public Health as a Home Services Agency (License #3003141) and provides non-medical adult home care services.
We are required by law to maintain the privacy and security of your protected health information, to give you this notice of our legal duties and privacy practices with respect to that information, and to follow the terms of the notice currently in effect. We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
We hold ourselves to HIPAA standards for every client we serve. Protecting your information is not a formality for us — it is part of how we earn the trust that lets us into your home.
Information this notice covers
To provide care, we create and keep records about you, including:
- Your name, address, phone number, and emergency contacts
- Your service plan and the schedule of care
- Notes made by caregivers about the assistance provided during each visit
- Information you or your family share with us about your needs, preferences, routines, and health conditions
- Billing and payment records
How we may use and disclose your information
To provide your care. We share your information with the caregivers assigned to you and with our office staff, so they can deliver and coordinate the services in your plan.
To coordinate with others involved in your care. With your permission, we may share information with your family members, your physician, a hospital discharge planner, a case manager, or a facility involved in your care.
For payment. We use your information to bill you or a third party responsible for payment, and to provide documentation a payer requires.
For our operations. We use your information to train and supervise caregivers, review the quality of our services, and run our business.
As required by law. We disclose information when Illinois or federal law requires it, including:
- Inspections, surveys, and investigations by the Illinois Department of Public Health
- Reports of suspected abuse, neglect, or exploitation of an adult or child, as required by the Adult Protective Services Act and related Illinois law
- Responses to a court order, subpoena, or other lawful process
- Public health reporting requirements
To prevent a serious threat. We may disclose information when necessary to prevent or lessen a serious and imminent threat to your health or safety, or to the health or safety of another person.
Uses that always require your written permission
We will not use or disclose your information for the following purposes without your written authorization:
- Marketing or promotional purposes
- Sale of your information — we do not sell your information
- Any purpose not described in this notice
If you give us written authorization, you may revoke it at any time in writing. Revoking it stops future uses and disclosures, but does not undo disclosures we already made in reliance on it.

Your rights
Access your records. You may ask to see and get a copy of the records we keep about you. We will respond within the time period required by law. We may charge a reasonable, cost-based fee for copies.
Request a correction. If you believe information in your record is incorrect or incomplete, you may ask us to correct it. We will explain in writing if we deny your request.
Request confidential communication. You may ask us to contact you in a specific way — for example, only at a certain phone number, or only by mail. We will accommodate reasonable requests.
Request restrictions. You may ask us to limit how we use or disclose your information. We are not required to agree to every request, but we will tell you if we cannot.
Get a list of disclosures. You may ask for a list of certain disclosures we have made of your information.
Get a paper copy of this notice. You may ask for a paper copy at any time, even if you have agreed to receive it electronically. Call us and we will mail one to you.
Choose someone to act for you. If you have given someone medical power of attorney, or if someone is your legal guardian, that person can exercise your rights and make choices about your information. We will verify their authority before acting.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with us. We will not retaliate against you in any way for filing a complaint, and it will not affect the care you receive.

Changes to this notice
We may change this notice, and the changes will apply to information we already have as well as information we receive in the future. The current notice will always be posted on this website with its effective date, and a copy is available at our office.
